ATTENTION!! IMPORTANT INSTRUCTIONS: If you need to complete new client paperwork then please click the link below associated with the type of counseling you need. Please keep in mind that you will need a different username/password for each type of counseling you may need (adult, child/adolescent or couples/marriage). If you need more than one type of counseling we suggest you use a prefix or suffix on your favorite username in order to differentiate each type of counseling. For example, if your username is johndoe and you need both adult and child counseling you would create two different usernames, johndoe_adult (for use in filling out adult paperwork) and johndoe_child (for use in filling out child paperwork). When completing the adult paperwork, you would go to our website and click on the "Individual Adult Counseling" paperwork button on our "New Client" page, create your johndoe_adult username and complete the adult counseling new client paperwork. Once complete, if you needed child counseling as well, you would go back to our website and click on the "Child Counseling" button , create your johndoe_child login and complete all child counseling new client paperwork. Each button on our website is unique to a specific set of paperwork. In other words, you can only fill out child counseling paperwork when you click on the "Child Counseling" button. You would not be able to fill out child counseling paperwork if you click on the "Couples/Marriage Counseling" button even if you entered your child counseling username/password.
